Waiver and Release Form
In plain terms you are agreeing to waive the right to hold Tee Off LLC responsible for any injuries occurred during or after using our facilities. Due to the nature of indoor golf, you are accepting the risks that can occur from being struck by a ball or club. No Alcohol is allowed in the in the facility. If you are caught with alcohol, you may be banned from the facility. You agree that any minors or guests brought into the facility have filled out this waiver and you will supervise them while they are on the premises. The following terms have been constructed by a legal team so you understand the risks involved and agree to use this facility responsibly and prioritize your safety and the safety of others.
I hereby acknowledge that myself and any parties brought into this facility may be seriously injured through the use of this facility. Golf balls are driven at high speeds, and golf clubs are swung at full speed, both of which reach speeds frequently exceeding 140 miles per hour; balls and tees may ricochet, and/or if you walk near someone swinging a club, you could get seriously injured. I understand that I must conduct myself in a controlled and reasonable manner at all times and to refrain from using any equipment in a manner inconsistent with its’ intended purpose and design. I agree to be solely responsible for the safety and wellbeing of my guests and myself. I understand that Tee Off LLC does not provide supervision, instruction, or assistance for the use of the facilities and equipment. Upon acknowledging that you may be injured, you agree that you are 100% at your own risk upon entering and using this facility, which means that you release Tee Off LLC of any and all liability. All participants in activities on the Premises, must agree to this waiver prior to entering. This Agreement shall remain valid and effective to release and indemnify Tee Off LLC (together with any of its managers, members, employees, independent and agents) from any Claims (as hereafter defined).
